Ausdance QLD periodically uses photographs, videos, profiles and stories for products that are promotional, educational, research or archival in nature. As such, Ausdance QLD collects on an ongoing basis individual and group photos and testimonials relating to our volunteers, supporters, participants and facilitators in activities. We ask for permission to use your photo, profile and/or story in promotional material to promote and benefit our projects and programs. "I hereby grant Ausdance Qld and its legal representatives' unrestricted permission to use and publish photographs or video images of me, or in which I may be included, for any purpose authorised by Ausdance Qld, including but not limited to: website use, editorial publications, documentation and advertising use. This includes the right to modify and retouch the images at the discretion of Ausdance Qld. I understand that I will not be given the opportunity to inspect or approve the finished products or the advertising copy or the printed matter that may be used in connection therewith. In granting this permission to Ausdance QLD and its legal representatives, I am fully and without limitation releasing it from any liability that may arise from the use of the images. I further agree to the inclusion of my name(s)."

Injury Waiver *

Ausdance QLD takes all reasonable care in the conduct of the classes and activities it runs and supports. The classes it is administering are run in professional settings by professional instructors. Ausdance QLD accepts no responsibility for injury or loss caused during any of its classes or events in any location. Dancers, facilitators, students and participants attending these activities are responsible for ensuring that they are physically and mentally fit for the practice undertaken and must at all times take care of their own personal safety. The participants, and for any participant who is under the age of 18, their guardian, warrants and certifies that, 1. The participant is fully responsible for the safety, management and care of their bodies whilst in the studio. 2. If the participant is currently injured or currently under any rehabilitation supervision for old, existing and/or re-occurring physical injuries or health conditions that might be affected by participation in these classes and activities, they do so at their own risk and within their limitations. 3. The participant is not the subject of an ongoing worker’s compensation claim, or if so please advise and acknowledge Ausdance QLD accepts no responsibility. 4. The participant will immediately inform the instructor of any physical difficulties or injuries that occur during the classes or activities. 5. The participant will report to Ausdance and fill out an incident report within two hours of any injury occurring. 6. Where the participant has exception to any of these warranties, the participant will speak to an administrative staff member prior to participating in the classes or activities
